I have had some henna peel right off, along with the skin, one time
when I used vinegar and lime cordial in my mix. I applied the henna
1 hour after initial mixing (!) and it turned cigar-wrapper dark. But
I think the very high acidity of that mix caused a little exfoliation
of the skin under the design. You may want to actually back off the
acidity of your mix a bit for that particular lady. She might have
acid sweat that, combined with the acidity of the mix, is chemically
removing the skin. Try more tea or coffee and less lemon, lime or
vinegar.
