More shipping woes- help?

package. It was mailed a couple weeks ago via USPS Priority Mail. I
am getting really fed up with this sort of thing, and I could really
use some advice on what to do now. This was a very large order which
I cannot afford to replace.
What are other people using for mailing? I'd say in the last year or
so, I've had maybe six people say their packages (about $125 total,
plus shipping) haven't arrived. Now I know to put delivery
confirmation on them, but that doesn't help them get there, only
tells me that they haven't yet. I've looked into FedEx and UPS and
the cost of shipping is almost double what the Post Office charges,
which means in some cases the shipping costs more than the product.
I hate to charge more than strictly necessary. Do higher shipping
charges mean that people order more at a time?
Also, how are other people setting up their shipping rates? Is it by
weight, or total dollar value, or a flat fee, and if flat fee, how is
