If the shop asks you to use waivers, I would do so; I think it's a good idea anyway. Some people may be allergic to terps in your mix (if you use them) and may not have known they had an allergy. I'd format the waiver, aftercare, and a section detailing what is in your mix all on one page. It's a good idea to have your terps listed in case there is a problem; that way they can take that information to their doctor so they'll get better treatment. Mine's a brochure-fold half letter page; it's easy to tuck them in my henna box. I'm redoing mine so that there's two copies of the waiver - one on the brochure for them, and a tear-off for me.
