First I ask people if they have ANY allergies or sensitivities no matter HOW WEIRD it sounds. If they say no, or if I approve the allergy, they read the waiver which explains the possibility of allergic reaction although I do use NO black henna and only natural ingredients et cetera, but that since it is a possibility, they understand that and I am not liable. Then there are about 20 lines per page underneath where they print and sign their names and write the date. It uses less paper and is less intimidating as a waiver. Everyone is fine with signing it, and I encourage them to thouroughly read the message on top (its a good idea for ANYTHING) it works well :-)you also get a record of how many customers you have had in a day.
