I've done both. Without a sealer, there's a chance that the lines will get fuzzy, depending on a few things....how fine the lines are, how much the area is banged around, and how warm/sweaty the area gets under the LL. It is apparently based on the fact that LL doesn't breathe...any sweat that comes up under it stays there. If excessive, and/or if the area is banged around, this can dislodge the LL where it is adhering to the skin, making more like a latex glove than a sealant. So the sweat/moisture can then fuzz out the lines. With NS applied first, it helps to affix the lines to the skin and makes them more resistant to the above effect. OTOH, with me it also prevents the ease in removal that I like so much with LL...that is, without a sealer, when you pull the latex off, it pulls almost all of the henna off with it. When using NS+LL, pulling the latex removes very little of the henna. I achieve a middle ground here by using lemonsugar to seal the design and apply the LL over that. It reduces the fuzziness effect, while still allowing the "one pull and it's off" feature. But it really just depends on how much sweating will be going on under the LL.
