Have you tried using a different brand of pure henna powder? Maybe
the brand you've been using has addditives, and they just aren't
listed? What color does the design come out on your skin (aside from
the rash)?

Do you use anything to cover/wrap the henna? Maybe the reaction is to
latex or New Skin if you're using that?
